    Wishing the injured worker a full recovery. Important that the investigation establishes what went wrong and whether procedures, training and equipment were adequate so similar incidents can be prevented. Clear communication with staff and families and a prompt review of risk assessments would help rebuild confidence.

It is right that an immediate health and safety investigation has been launched to establish what happened and whether safe systems of work were in place. The priorities now should be ensuring the site is secured, staff are supported, and any witnesses are assisted by investigators. Findings need to be shared promptly and any identified remedial actions implemented to prevent a repeat.

An immediate, thorough investigation is the right response to establish what happened and to prevent a recurrence. Key priorities now should be securing the scene, preserving evidence, supporting the injured person and their colleagues, and reviewing risk assessments and procedures. Springfield should ensure full cooperation with investigators, preserve the scene and any relevant equipment and provide support for staff affected by the event. Once the facts are known lessons should be shared to prevent recurrence and any necessary changes to procedures or training implemented without delay.
