In short: lock out tag out (LOTO) means isolating every energy source on a machine, locking the isolation with a personal lock, tagging it, releasing stored energy and proving the equipment dead before anyone puts a hand inside. There is no single UK LOTO regulation: the duty comes from PUWER 1998 (regulations 11, 19 and 22), the Electricity at Work Regulations 1989 (regulations 12 and 13) and the Health and Safety at Work etc. Act 1974. A tag alone, an interlocked guard or an emergency stop is not isolation.

Why LOTO matters

Most machinery injuries happen not in normal production but when a machine is being cleaned, unblocked, adjusted or repaired, and somebody else assumes the job is finished. In 2024/25 HSE’s provisional RIDDOR figures record 13 workers killed by contact with moving machinery in Great Britain and 2,088 non fatal injuries to employees from the same cause.

Behind almost every one of those incidents is the same short story: the equipment was stopped but not isolated, isolated but not locked, or locked but never proved dead. LOTO closes those three gaps with a physical padlock rather than a promise.

What LOTO is, and what it is not

LOTO is a system of work, not a piece of kit.

  • Lock out means placing a physical lock on an energy isolating device (a breaker, a valve, a blanking plate) so that it cannot be operated until the lock is removed. The person doing the work holds the only key.
  • Tag out means attaching a written warning to the isolation point stating who applied it, when and why. A tag informs; it does not prevent.
  • Try out (proving dead) means attempting to start the equipment, or testing with a suitable instrument, to confirm the isolation has worked. Without this step, LOTO is a ritual.

British practice uses the same principles under the headings “safe isolation” and “lock off”. HSE’s maintenance guidance is blunt: plant and equipment must be made safe before maintenance starts, and machines must be locked off if there is a chance the power could be accidentally switched back on. Pressing an emergency stop, opening an interlocked guard or switching off at a control panel does not separate the equipment from its supply and is not isolation.

The law: where the UK duty comes from

There is no regulation in Great Britain called “lock out tag out”. The requirement is assembled from several instruments.

Health and Safety at Work etc. Act 1974

Section 2 requires every employer to ensure, so far as is reasonably practicable, the health and safety of employees, including safe systems of work. Most isolation prosecutions are brought under section 2(1).

Provision and Use of Work Equipment Regulations 1998 (PUWER)

  • Regulation 19, isolation from sources of energy. Every employer must ensure that, where appropriate, work equipment is provided with suitable means to isolate it from all its sources of energy. Those means are not suitable unless they are clearly identifiable and readily accessible. The employer must also take appropriate measures to ensure that reconnection of any energy source does not expose anyone to risk. Note “all its sources”: the duty does not stop at electricity.
  • Regulation 11, dangerous parts of machinery. Measures must prevent access to any dangerous part of machinery, or stop its movement before any part of a person enters the danger zone, following the hierarchy of fixed guards, other guards or protection devices, jigs and holders, then information, instruction, training and supervision.
  • Regulation 22, maintenance operations. Work equipment must, so far as is reasonably practicable, be constructed or adapted so that maintenance involving a risk can be carried out while the equipment is shut down, or otherwise without exposing the person to risk.

Electricity at Work Regulations 1989 (EAWR)

  • Regulation 12, means for cutting off the supply and for isolation. Where necessary to prevent danger, suitable means (including methods of identifying circuits) must be available for cutting off the supply to any electrical equipment and for isolating it. Isolation is defined as disconnection and separation from every source of electrical energy in such a way that the disconnection and separation is secure. “Secure” is why a padlock matters.
  • Regulation 13, precautions for work on equipment made dead. Adequate precautions must be taken to prevent equipment that has been made dead from becoming electrically charged during the work if danger may arise.

HSR25 is the HSE guidance on the regulations. HSG85, Electricity at work: safe working practices (third edition), sets out the safe isolation sequence for electrical work, including proving dead with an instrument that is itself proved before and after use. For process plant, HSG253, The safe isolation of plant and equipment, was written for oil, gas and chemical operations but is the reference point for any site with pressurised or hazardous systems.

Energy sources, isolation points and proving methods

The most common failure in a written procedure is that it lists the electrical breaker and nothing else.

An isolation is only as good as its weakest energy source: a locked breaker does nothing about a platen held up by a leaking hydraulic valve.

The isolation procedure step by step

  1. Prepare. Identify the task, the machine and every energy source that feeds it, and gather locks, hasps, tags and test equipment. If any source has no lockable isolation point, stop and escalate.
  2. Notify. Tell the operator, the supervisor and anyone else affected, including upstream and downstream teams on a shared line.
  3. Shut down. Stop the machine by its normal method so it comes to rest in a controlled state.
  4. Isolate. Open breakers, close valves, disconnect plugs, insert spades. Isolate at the supply, not at a control switch.
  5. Lock and tag. Apply a personal lock to each isolation device (a multi hasp where several people are involved) and a tag showing name, date, time and task. Each person keeps their own key. Isolation locks should be a distinct colour and never used for anything else.
  6. Release stored energy. Bleed pressure, discharge capacitors, lower or prop suspended parts, relax springs, let hot parts cool. Check gauges read zero.
  7. Prove dead, try out. Attempt to start from every start point, including remote and automatic starts, then return the controls to off. For electrical work, test for voltage with a proved instrument. If anything moves or reads live, go back to step 4.
  8. Work. If the person changes, the lock changes. If the task changes, reassess the isolation.
  9. Restore. Remove tools, refit guards, clear the danger zone, confirm the lock count matches the people count, remove locks (each person their own), notify affected staff, and re energise in a controlled sequence with a test run before hand back.

Never remove another person’s lock. If a lock holder has gone home with the key, the only acceptable route is a documented, senior authorised removal: confirm by telephone that the person is off site, inspect the equipment, record the removal with two signatures, and tell the lock holder before they next start work. Bolt cutters in a supervisor’s drawer are an audit finding, not a control.

Group lockout and lock boxes

When several people work on one machine, or a machine has a dozen isolation points, personal padlocks on every device become unmanageable. The answer is group lockout.

  • An authorised person isolates every energy source, places a departmental lock on each point and records it on an isolation certificate.
  • The keys to those locks go into a lock box.
  • Every person working on the machine places a personal lock on the lock box hasp, so nobody can retrieve the isolation keys until every personal lock is off.
  • Each worker still proves the isolation for themselves, or witnesses the authorised person doing so.

Group lockout gives you one place to count locks against people, which is exactly what fails in most restart incidents. Keep the lock box at the machine.

Permits to work versus LOTO

  • LOTO is the physical control that makes the equipment safe. It applies to every intervention that exposes someone to an energy source, including a five minute blockage clearance.
  • A permit to work is a formal, documented authorisation for higher risk work, recording hazards, precautions (including isolations), authorisation, time limits and hand back. HSE’s maintenance guidance notes that formal systems such as a permit to work are required in some cases to safely manage high risk maintenance.

Rule of thumb: LOTO always; a permit for hot work, confined space entry, pressurised or hazardous process systems, contractors on live plant, or isolations shared across teams. Special cases

Contractors

Contractors do not know your plant. Induct them on the site procedure, show them the isolation points for the equipment they will touch, and require personal locks alongside your departmental lock. Never let a contractor rely on an operator saying “it’s off”. See managing contractors under CDM 2015.

Shift changeover

A task that runs past the end of a shift is a classic restart trap. The outgoing worker must not remove their lock until the incoming worker has applied theirs, or a departmental lock bridges the gap under the supervisor’s control. Record the handover on the isolation certificate.

Interlocked guards and why they are not a substitute

Interlocked guards satisfy PUWER regulation 11 during normal operation. But an interlock is a control circuit function, not an isolation: it can be defeated with a spare actuator, bypassed for fault finding and never reinstated, it does nothing about hydraulic, pneumatic or gravitational energy, and anyone closing the guard resets it. Use interlocks only for the minor routine tasks the machine’s risk assessment specifically allows behind them. For anything that places a body part where a restart would injure it, isolate and lock.

Equipment that cannot be locked, and equipment that starts itself

Retrofitting lockable isolators or plug lockouts to older machines is cheap; tag only isolation should trigger a purchase order, not a permanent workaround. At the other extreme, robot cells, PLC controlled lines and timed equipment can start without anyone pressing a button, so proving must include waiting through the cycle.

Case study: a restart that should never have happened

On 24 March 2023 an employee of Stonegrave Aggregates Limited, part of the Ashcourt Group, was cleaning a waste picking line at Aycliffe Quarry in Aycliffe Village, County Durham. A senior supervisor restarted the machinery, having mistaken the worker for a colleague who was wearing similar clothing. The employee was trapped and suffered a fractured shoulder, torn ligaments and a broken finger.

HSE’s investigation found that the company failed to ensure an isolation procedure was correctly implemented while employees were cleaning machinery. The company pleaded guilty to breaching section 2(1) of the Health and Safety at Work etc. Act 1974 and was fined £270,000 with £15,637 in costs at Teesside Crown Court in October 2025. HSE noted that a 2015 incident at the same site, in which one worker died and others were seriously injured, had also involved isolation failures. The inspector said incidents like this can and should be prevented by following robust lock off procedures, and that effective monitoring, auditing and review of isolation systems is essential to demonstrate that they are working.

Cleaning is maintenance for isolation purposes, and “I thought it was someone else” only becomes possible when a restart depends on human recognition rather than on a padlock that the worker alone can remove.

Common mistakes

  • Treating a blockage as too quick to isolate. Most trapping injuries happen during short interventions.
  • Isolating at the control panel. A stop button, a key switch or a touchscreen “off” is not disconnection from the supply.
  • One lock for the whole team. If the fitter’s lock protects the electrician, the electrician is unprotected the moment the fitter finishes.
  • Skipping the try out. Isolators are mislabelled more often than anyone expects.
  • Forgetting gravity and pressure. Platens, hoppers, tipping bodies and accumulators injure people on machines whose electrical isolation was perfect.
  • Auditing paperwork instead of practice. Completed certificates prove nothing unless someone watches an isolation being done.

Frequently asked questions

Is LOTO a legal requirement in the UK?

The words are not, but the substance is. PUWER 1998 regulations 19 and 22, the Electricity at Work Regulations 1989 regulations 12 and 13, and section 2 of the Health and Safety at Work etc. Act 1974 together require secure isolation from every energy source, precautions against re energising and a safe system of work. A padlocked, proved isolation is how those duties are met.

Can we use tags only, without locks?

Only where the device physically cannot accept a lock, and then with extra measures such as removing a fuse or plug and keeping it with the worker. If your isolators cannot be locked, fit ones that can.

Who is allowed to remove a lock?

Only the person who applied it. Forced removal in their absence is permitted only under a written procedure authorised by a named senior manager, after confirming the person is clear and inspecting the machine, and must be recorded and reviewed as a near miss.

Does opening an interlocked guard count as isolation?

No. An interlock is a legitimate control for the specific minor tasks the risk assessment allows behind it, but it does not disconnect the supply, does not deal with stored or gravitational energy, and can be reset by anyone. If a restart would injure the person, lock off.

How do we prove dead on electrical equipment?

Follow HSG85: after isolating and locking, prove a suitable voltage indicator on a proving unit, test between all conductors and to earth, then prove the indicator again. A non contact voltage pen is not adequate.

Do operators need LOTO training or just maintenance staff?

Anyone who may reach into a machine, including operators clearing blockages and cleaners, must be either trained and authorised to isolate or trained to stop and call someone who is. Everyone else needs to know what a lock and tag mean and that they must never be touched.
