Expectant mothers in the south-west of England are facing significant uncertainty due to the unexpected closure of a local maternity unit, prompting them to prepare for the possibility of giving birth en route to hospital. As a result of the closure at North Devon District Hospital in Barnstaple, which has been attributed to chronic staffing shortages, all childbirth services, including cesarean sections, were suspended as of 11 August. This situation has led to increasing anxiety among pregnant women in the area.

With a maternity ward now closed, women scheduled to give birth are being advised to take precautions, such as bringing along blankets, towels, and large bottles of water when travelling to their alternative maternity hospitals. Furthermore, they are instructed to ensure their vehicles have sufficient fuel in order to avoid being stranded during this critical time. There are reports that, in response to this crisis, expectant parents are being provided with classes on how to safely deliver a baby in a car, even on the side of the motorway.

Catherine Mchomvu, an antenatal educator who leads these sessions, explained, “It’s about trying to create the birth environment that you would normally have in the hospital or at home, in your car.” She emphasises that for some women, labour can progress rapidly, so she instructs them on positions to manage contractions, essential items to have on hand, and the importance of delaying cord cutting until medical assistance arrives. Mchomvu points out that expectant mothers are already under immense stress and deserve reassurance, especially at a time when anxiety surrounding childbirth should ideally be tempered by confidence in available care.

The North Devon healthcare trust has confirmed that it has also suspended triage appointments, which are designed to monitor the health of expectant mothers and their unborn children, compounding the fears of those awaiting delivery.

Hannah Rulton, a mother of two currently 27 weeks pregnant, resides in Ilfracombe, where she faces the daunting prospect of a journey of over an hour and a half to reach the Royal Devon and Exeter Hospital. The 37-year-old recounts distressing experiences from her previous labor, saying, “My son was born with shoulder dystocia – which caused his shoulder to get stuck on my pelvic bone – and it took five minutes to resuscitate him.” She fears that history may repeat itself with her forthcoming birth, as the risk of shoulder dystocia is heightened for her. Rulton expresses her concerns: “If I go into spontaneous labour, which is likely, we’ll be in a car for at least an hour and 40 minutes on our own without any help or intervention that we might need if it happens on the side of the road.” Describing her emotional turmoil, she adds, “This special time is being distorted because I’m constantly worrying and thinking, ‘what if the worst happens?’”

Similarly, Vicky O’Loughlin, 34, who is expecting her child in October, voiced her distress following the announcement of the ward closure. Reflecting on her feelings, she stated, “When I first learned of the closure I was really shocked and upset. My family are all from here [Barnstaple] and I was born here. I’m really stressed.” She is particularly concerned about her health, having a condition that affects her immune system and could potentially increase her risk for pregnancy complications like preeclampsia. O’Loughlin mentions receiving limited guidance from hospital staff, stating, “They’ve given me leaflets and some advice from the midwife, but that’s all. We don’t have anywhere to go and we can’t afford accommodation in Exeter.”

In light of these developments, Ian Roome, the Liberal Democrat MP for North Devon, has raised concerns regarding the sustainability of the current situation. “Tens of thousands of people rely on NDDH,” he emphasised, highlighting the inadequacy of asking patients to travel significantly further for essential maternity care. Roome deems the health trust’s response unacceptable, arguing it compromises the safety and wellbeing of pregnant women and newborns. He has reached out to Health and Social Care Secretary Yvette Cooper to urge for increased funding for rural acute hospitals.

Responding to the crisis, Sam Higginson, Chief Executive of the North Devon healthcare trust, acknowledged the seriousness of the situation, stating, “We’re trying to get it reopened as quickly as we can. It is totally dependent on us being able to attract both permanent and temporary staff.” This sentiment comes at a time when healthcare services are under greater scrutiny to maintain quality care amidst staffing challenges.

A spokesperson for the government reiterated the commitment to ensuring maternity care is safe and accessible. “We expect clear plans to be in place to ensure mothers and babies are supported during this temporary closure, and for services to be back up and running as soon as possible,” the spokesperson said. They also pointed to the record numbers of NHS staff recruited, including over 12,000 doctors and an additional 2,000 midwives, affirming that the government is dedicated to improving maternity services throughout the country, including in Devon.

As the community grapples with the fallout of the maternity unit’s closure, the concerns of expectant mothers highlight the urgent need for solutions to ensure safe, reliable care during a time when it is needed most. The emotional toll on these individuals serves as a powerful reminder of the critical nature of accessible healthcare for families during the perinatal period.

Ultimately, prioritizing the maternity workforce and infrastructure can ensure that healthcare remains accessible and safe for all women, reducing the likelihood of life-threatening situations during labor.
