Fears have escalated regarding a potential plague outbreak in Russia following the death of a researcher at a facility dedicated to handling dangerous pathogens. Darya Shipilova, 28, who was employed at the Irkutsk Anti-Plague Research Institute in Siberia, succumbed to severe pneumonia on October 2. Reports suggest that she may have had contact with live plague bacteria shortly before her illness.

Russian officials have not confirmed any outbreak but described Shipilova’s illness as pneumonia of unknown cause. In response to her death, authorities have placed nearly 200 individuals who may have been in contact with her under medical observation, with additional precautions implemented at multiple hospitals. It remains uncertain whether any further cases of illness have been reported.

The threat is centred around pneumonic plague, caused by the bacterium Yersinia pestis, historically known for its role in the Black Death. The UK Health Security Agency defines symptoms of pneumonic plague, which can include fever, headache, weakness, and rapidly progressing pneumonia, producing shortness of breath, chest pain, and bloody or watery mucus. Unlike the bubonic form, pneumonic plague can transmit between humans via respiratory droplets, necessitating close contact for transmission.

Symptoms typically appear within one to seven days after exposure, and untreated pneumonic plague is often fatal. However, antibiotics can effectively treat the infection if administered promptly.

Despite the alarming situation, the probability of the plague spreading to the UK is regarded as very low by health officials. The UK Health Security Agency has stated that the plague is no longer endemic in Britain, and the risk posed by an infected traveller returning is minimal.

Dr Simon Clarke, an associate professor in cellular microbiology at the University of Reading, cautioned against drawing parallels between pneumonic plague and the Covid-19 pandemic. He emphasised the differences in transmission, noting that pneumonic plague is not as contagious as viral respiratory illnesses.

He addressed the concerns surrounding the lab worker’s death, advising that multiple safeguards are in place in high-containment laboratories to prevent the escape of pathogens. Dr Clarke expressed scepticism about the likelihood of a lab-acquired infection occurring under standard safety protocols, stating that “a lot must have gone wrong for a lab worker to be infected by a broken test tube.” He indicated the need for more information to determine whether this incident is an isolated occurrence or indicative of a broader transmission risk.

Additionally, ensuring that incident reporting mechanisms are in place in accordance with RIDDOR 2013 will help facilitate immediate responses and improve overall laboratory safety standards.
