In Lincoln, a 27-year-old woman named Caity Duggan has faced a significant challenge in accessing dental care through the National Health Service (NHS) for the past six years. Despite numerous attempts to secure an appointment at her local dental practice in Brigg, she was informed that her dentist had left and was not assigned to another practitioner. Duggan explained, “They weren’t willing to see me unless I paid to go private.” With financial constraints making private dental care unattainable, she endures ongoing pain from her wisdom teeth.

Duggan’s situation highlights a troubling trend in England, as numerous areas are experiencing the emergence of “dental deserts.” These are regions where NHS dental services are increasingly scarce, largely due to many dental practices opting out of NHS contracts. Critics have pointed to inadequate funding as a primary reason for this shift, which has left patients with limited options—either face unmet dental needs or pay out of pocket for private care, which is often prohibitively expensive.

The British Dental Association (BDA) recently reported a deterioration in access to dental services across various parts of the country. It indicated that circumstances have worsened significantly, with many practices exiting the NHS system in favour of more profitable private patients. Since 2017, over 600 dental clinics in England have ceased providing NHS services, according to an analysis by the Nuffield Trust.

Lincolnshire, where Duggan resides, is among the most severely affected areas. Currently, only seven practices offer NHS dentistry per 100,000 residents—a figure that falls below the national average of ten per 100,000. New patients struggle to find appointments; only 28% are able to secure an initial visit, starkly at odds with the England average of 38%.

Duggan, who works as an employee relations adviser, is particularly anxious about the mounting dental costs she may face after this prolonged absence of care. Having been quoted hundreds of pounds just for a private consultation, she reflects on the unfortunate realities of her situation: “No one’s taking on new patients because no one has the dentist with any spare spaces.” She fears that when she is finally able to see someone, she will face a lengthy list of dental issues that need attention, for which she will be financially liable, despite the circumstances being beyond her control. Additionally, she worries about the potential for infection due to her untreated pain.

The NHS Lincolnshire Integrated Care Board has acknowledged unmet oral health needs in regions such as Lincoln, Boston, and Skegness, where health indicators concerning dental health are particularly poor. To help address these challenges, the University of Lincoln has launched a campaign to establish the first dental school in the county. The aim is to train more students locally, thus increasing the likelihood that graduates will remain in the area to practice.

This initiative has garnered support from local political figures, including Martin Vickers, the MP for Brigg and Immingham. He emphasised the importance of the project, declaring it “critical” for enhancing access to NHS dental services in Lincolnshire. However, critics caution that while developing local training programs could help in the long term, immediate reforms are needed to retain those already in the profession.

Dr. Shiv Pabary, chair of the BDA’s General Dental Practice Committee, noted that the establishment of a nearby dental school could lead to increased availability of dental care for residents. He mentioned that graduates from dental schools tend to stay in their locality, thereby alleviating some of the access issues faced by patients.

Nevertheless, he underscored the essential need for the government to act swiftly to remedy the NHS dental contract, which many argue is inadequate for current demands. Dr. David Trawford, a dentist who transitioned to private practice 23 years ago, shared his experience regarding the fundamental changes in NHS funding, stating, “I do know of a practice in Spalding that used to provide NHS treatment. They have now gone private, and it will be in all cases for the same reason.” Many of his colleagues are making the similar switch away from NHS contracts due to financial pressures.

The dental crisis in England is reflected in the experiences of individuals like Duggan and organisations like Toothless in England, founded by Mark Jones. This grassroots campaign draws attention to alarming issues, including reports of individuals resorting to self-extraction and children requiring hospital admissions for dental procedures—pointing to a nation in crisis regarding oral health.

The NHS Lincolnshire Integrated Care Board was contacted for comment but has yet to provide a response regarding this urgent situation and ongoing efforts to bolster dental services across the region. As the challenges mount, patients continue to seek solutions amidst a landscape of diminishing dental health provision, making the need for immediate action and reform more pressing than ever.

Furthermore, addressing local discrepancies in dental service availability will reduce the risk of untreated dental issues that can lead to more severe health complications.
